Health Navigation
Health Navigation is an integral part of Lumia Health. A combination of technology and Lumia Health personnel will help patients access services and care from tenant partners. With personalized guidance through the process, patients will confidently navigate the Lumia ecosystem to improved health outcomes for themselves and Nova Scotia’s healthcare system.
Proactive Emphasis
Lumia Health is designed to complement the primary care system by providing Nova Scotians with an avenue to be proactive with their health and avoid adding strain to the acute care system. Lumia Health will invite a community of like-minded tenant partners to thrive while serving patients who are taking control of their health by participating in a multidisciplinary health ecosystem.
Technology Driven
Lumia Health will place a heavy emphasis on innovation and technology, removing some of the traditional silos within healthcare. Patients will access a personal portal and be an active participant in the process. With their consent, true collaboration will be achieved when their records and health history are accessed by tenant partners working with the patient.
